Practice Financial Policy

In order to give you the best service possible and to assist you in the understanding of our financial policies, we feel that the following information will be helpful.

Fees:
Information concerning our fees is available upon request. If you should require special testing, such as hearing test or allergy testing, etc, there will be additional charges. You are encouraged to inquire about the cost of any procedure before it is performed.

Before any surgery is scheduled, the doctor's charge and the payment method will be discussed with you by one of our insurance representatives. Your acceptance of this charge will be noted in your record. Surgery will then be scheduled and carried out as planned.

It is requested that the charges be paid at the time our services are given. We accept cash, check and most credit cards. If payment of the charges creates a financial burden for you, please tell us so that special arrangements for payment can be made.

Insurance:
Health insurance is an agreement between you and the insurance company to provide specified benefits for surgery or medical treatment.

Surgical benefits paid by the insurance company rarely cover the full amount of your surgery. Therefore, before your surgery is performed, a partial payment is required to cover the difference not paid by insurance. Any overpayment will be refunded promptly.

Please bring your insurance cards with you on each visit. If you have insurance with a managed care company with which we have a contract, you will be responsible for paying the co-pay of co-insurance at the time of your visit. If you do not have your insurance card, you will be expected to pay the charges for that visit. If your insurance company requires a referral from your Primary Care Physician, please contact them for the referral before your visit to our office.

We are in contract with many insurance plans. We also accept Medicaid and Medicare.

Past Due Accounts:
Past due accounts that cannot be collected by our office are turned over to an agency for collection. We encourage anyone who has difficulty paying an account to discuss this with one of insurance representatives. A payment schedule can be set up to pay off the account. This avoids the unpleasant action of having a collection agency involved.
